Team Requirements

Our roster needs to be finalized before next Tuesday, January 24th. Here is what we need from you to be on our team this year:
  1. general info: your name, student number, contact info, year, and previous lax position if applicable
  2. club sport dues: $50, which will be added to your tuition unless you submit a check made out to Calvin College
  3. lacrosse insurance: $50, you need to sign up for this (or renew your membership) through US Lacrosse (http://www.lacrosse.org/membership/index.phtml) and we need a copy as proof to keep on file

You may email me any of this information or submit it at practice, but please note that the deadline is fast approaching.
